Penn Medicine is currently engaged in the FY2019 Community Health Needs Assessment (CHNA) process, which will inform our Community Health Implementation Plan (CHIP). In order to maximize impact with our community health initiatives, it is necessary to first understand the current state of activity. We know there is a wealth of great work going on across UPHS and the Perelman School of Medicine, and we want to make sure we capture it all.

To assist us in canvassing our system for community health outreach programs, please do the following:

1.    Complete the survey once for each community health program/initiative you supervise

2.    Share the survey widely with your colleagues

We appreciate your efforts in helping us establish an inventory of Penn Medicine’s community health initiatives. Data from the survey will be used to support quality improvement and strategic planning efforts for Penn’s future CHNAs/CHIPs.
